Types of pain:

Please arrive 30 minutes prior to your first appointment so we can prepare your chart.

  • Insurance card
  • Picture ID
  • Co/pay co/insurance
  • Films (i.e. MRI, XRAYS) The actual film, not just the report

Ideally, you should have your recent x-rays and or MRI’s for the doctor to view.

All workers’ compensation and no-fault patients should have ALL their case and policy numbers and the name and phone number of a contact person. You must also bring in your private insurance information to be placed on file and a referral if applicable.

Please arrive on time. For confirmation, we will call you 24-48 hrs. prior to your appointment If you are not available, but have an answering machine, we will leave a message. If we cannot contact you for any reason, i.e. your phone is disconnected; we will cancel your appt. and fill that time with another patient.

You may be wondering what to expect at your first appointment. You’ll have a 1-hour time slot. You can expect to discuss your medical history and the pain that you are experiencing. Then, your doctor will do a physical exam and talk briefly about a treatment plan for you.

Before your appointment, please make sure to fill out the New Patient Questionnaire. You can find it in the Forms section on our website. If you can’t fill it out online, we can send you a copy by mail. Remember, it’s important to complete the questionnaire before you come in for your appointment.

Don’t forget to fill out the patient questionnaire completely before you come. We want to make sure we have all the information we need to give you the best care possible.

Yes, you can be referred to ASCPM by your doctor for an injection only.  Your doctor needs to send a prescription that says what injection they want you to have, along with your insurance details and any pre-authorizations or pre-certifications required. The injection will be done in our office, and we’ll use x-ray guidance to make sure it’s done accurately. After the procedure, remember to schedule a follow-up appointment with the doctor who referred you.

ASCPM accepts many medical insurance carriers. If you don’t see your insurance company, please call our office!

Office Hours: Our centers are open Monday through Friday, from 8:30 AM to 4:30 PM. Please call our office today to make an appointment for a consultation.

To keep things safe, certain medications are controlled, which means you need to see us regularly for prescription refills. We can’t refill or prescribe these controlled substances after hours, on weekends, or on holidays. Also, we can’t provide prescription refills at surgical facilities on the day of your procedure. It’s important to plan ahead and make sure you have enough medication to cover these times. If you have any concerns or questions, please reach out to us during regular office hours.
